Evidence-Based Hair Loss Treatments: What Actually Regrows Hair
A research walkthrough of the treatments with the strongest clinical evidence for slowing and reversing pattern hair loss, including minoxidil, finasteride, microneedling, PRP, and ketoconazole.

Androgenetic alopecia, the technical name for common male and female pattern hair loss, is largely a genetic condition, and no treatment on the market reverses it completely or works for everyone. That is not a reason to give up on treatment, it is a reason to be precise about what the clinical evidence actually shows. A handful of interventions have been tested in randomized, placebo-controlled trials with hundreds to thousands of participants and consistently demonstrate the ability to slow progression and, in a meaningful share of users, produce visible regrowth. Others have promising early data but smaller or lower quality trials, and some popular products have little to no controlled evidence at all. This article covers treatments in roughly descending order of evidence quality: mechanism, key studies with real participant numbers and effect sizes, time-to-result, side effects, and who should avoid each option. None of this replaces seeing a dermatologist, particularly before starting any prescription medication.
The biology: why follicles shrink in the first place
Pattern hair loss follows a process called follicular miniaturization. In genetically susceptible follicles, the enzyme 5-alpha-reductase converts testosterone into dihydrotestosterone (DHT), a more potent androgen that binds receptors in the follicle's dermal papilla. Over repeated hair cycles, that binding progressively shortens the anagen, or growth, phase and lengthens the resting phase, so each new hair is finer, shorter, and less pigmented, until the follicle produces only a fine vellus hair or nothing visible at all. That is why pattern hair loss looks like gradual thinning rather than sudden patchy loss.
Two other factors matter alongside DHT sensitivity. Blood flow to the scalp delivers oxygen and nutrients follicle stem cells need to cycle through growth phases, part of why vasodilating treatments have real mechanistic rationale. Tier 1: The strongest evidence, FDA-approved options
Minoxidil (topical and oral)
Topical minoxidil is the only over-the-counter medication approved by the FDA for pattern hair loss in both men and women. Its mechanism is not fully settled, but the leading explanation combines vasodilation, which increases blood flow and oxygen delivery to the follicle, with a direct effect on potassium channels that extends the anagen growth phase and enlarges miniaturized follicles. It does not block DHT.
Low-dose oral minoxidil (typically 0.25 to 5 mg per day, far below the 10 to 40 mg used for blood pressure) has emerged as an off-label alternative for people who cannot tolerate topical formulations. A comprehensive review found response rates ranging from 61 to 100 percent of patients showing objective clinical improvement across pooled trials (Skin Appendage Disorders, 2023), and a dedicated trial in female pattern hair loss reported meaningful increases in total and terminal hair counts after 24 weeks (PubMed, 2020). A multicenter safety study of 1,404 patients found systemic adverse effects were infrequent, with only 1.7 percent discontinuing treatment because of them, though hypertrichosis (unwanted facial or body hair) occurred in roughly 15 percent of users (Journal of the American Academy of Dermatology, 2021).
Across both formulations, roughly 30 to 40 percent of users see moderate to significant regrowth, a larger share see stabilization, and a minority see no response. Meaningful change typically takes 4 to 6 months, and temporary increased shedding in the first 2 to 8 weeks is expected as the hair cycle resets, not a sign of failure. Topical minoxidil is available without a prescription; oral minoxidil for hair loss is off-label and should only start after a conversation with a doctor, particularly for anyone with low blood pressure, heart disease, or fluid retention.
Finasteride (oral and topical)
Finasteride selectively inhibits type II 5-alpha-reductase, lowering scalp and serum DHT by roughly 60 to 70 percent and removing much of the signal driving miniaturization. It is prescription-only, FDA-approved for men, and not approved for women of childbearing potential due to birth-defect risk in a male fetus.
The landmark trial is the Finasteride Male Pattern Hair Loss Study Group's 1998 publication, which followed 1,553 men aged 18 to 41 in two one-year, placebo-controlled trials, with 1,215 continuing into blinded extension studies for a second year. Finasteride-treated men gained an average of 107 additional hairs at one year and 138 at two years in a standardized scalp area, versus progressive hair loss on placebo (Journal of the American Academy of Dermatology, 1998). A follow-up analysis found finasteride reduced the five-year likelihood of further visible hair loss by 93 percent relative to placebo (European Journal of Dermatology, 2008). A 2010 systematic review pooling 12 randomized trials and 3,927 men found finasteride increased self-reported improvement (relative risk 1.81 short term, 1.71 long term) and hair count by a mean of 9.4 percent short term and 24.3 percent long term versus placebo (Archives of Dermatology (JAMA Dermatology), 2010).
That same review found a modest increase in erectile dysfunction (relative risk 2.22, number needed to harm about 82). A small subset of men report persistent sexual, cognitive, or mood-related symptoms after stopping, sometimes called post-finasteride syndrome, whose mechanism remains debated. Anyone considering finasteride, especially with a personal or family history of depression, should discuss the full risk profile with a physician first, and stop and consult a doctor promptly if mood or sexual side effects appear. Topical finasteride, aiming for lower systemic absorption, is increasingly prescribed off-label but has a smaller trial base than the oral form.
Dutasteride
Dutasteride inhibits both type I and type II 5-alpha-reductase, lowering DHT by around 90 percent versus finasteride's 60 to 70 percent. It is FDA-approved for prostate conditions but used off-label for hair loss. In a 24-week, placebo- and finasteride-controlled dose-ranging trial of 416 men, dutasteride increased hair counts dose-dependently, and the 2.5 mg dose outperformed finasteride 5 mg at 12 and 24 weeks by expert panel photographic assessment (Journal of the American Academy of Dermatology, 2006). Head-to-head studies generally confirm dutasteride's superior efficacy. That greater potency brings a comparable or slightly higher rate of sexual side effects, a longer half-life, and reduced sperm count relevant to men trying to conceive. It is generally reserved for men who have not responded to finasteride and should only be used under dermatologist supervision.
Tier 2: Strong supporting evidence
Microneedling
Microneedling uses a roller or pen studded with fine needles to create controlled micro-injuries in the scalp. The wound-healing response releases growth factors and activates dermal papilla stem cells, and the micro-channels temporarily improve penetration of topical medications like minoxidil. In Dhurat and colleagues' 2013 randomized, evaluator-blinded study of 100 men, twice-daily 5% minoxidil alone was compared against the same regimen plus once-weekly microneedling for 12 weeks. The combination group gained a mean of 91.4 hairs/cm2, roughly four times the 22.2 hairs/cm2 gained by minoxidil alone, and 82 percent reported at least 50 percent improvement versus 4.5 percent on minoxidil alone (International Journal of Trichology, 2013). A broader review of 17 studies covering more than 900 patients found microneedling consistently improved outcomes paired with minoxidil, growth-factor solutions, or PRP, with no serious adverse events (Dermatology and Therapy, 2021). Typical protocols use 0.5 to 1.5 mm needle depth weekly to biweekly; side effects are limited to transient redness, mild discomfort, and rare pinpoint bleeding.
Platelet-rich plasma (PRP)
PRP involves drawing a patient's own blood, concentrating the platelets by centrifuge, and injecting the plasma into the scalp, where growth factors are thought to prolong the anagen phase and stimulate dermal papilla activity. A 2019 systematic review and meta-analysis pooling data across multiple trials found a standardized mean difference in hair density of 0.58 favoring PRP versus baseline and 0.51 versus placebo injections, both statistically significant (Gupta et al., Dermatologic Surgery, 2019). Other reviews flagged heterogeneity in how PRP is prepared and administered, with at least one finding the evidence generally positive but not yet definitive (Skin Appendage Disorders, 2019). The standard protocol is three monthly sessions followed by maintenance every 3 to 6 months. PRP is an off-label use of an autologous blood product, and cost (typically several hundred dollars per session, rarely covered by insurance) is the main practical barrier.
Ketoconazole shampoo (2%)
Ketoconazole is an antifungal sold over the counter as a dandruff shampoo (Nizoral) at 1% strength, with a 2% version available by prescription in some markets, and it has mild anti-androgenic and anti-inflammatory effects at the scalp. In a study comparing 2% ketoconazole shampoo to 2% topical minoxidil and a placebo shampoo, ketoconazole produced increases in hair density and anagen-phase follicles comparable to minoxidil and clearly better than placebo over six months (Perez, Journal of Dermatological Treatment, 2004), and a separate long-term study found sustained benefit with continued use (PubMed). It is inexpensive and well tolerated, a reasonable low-cost adjunct, though not a primary treatment on its own for moderate to advanced hair loss.
Tier 3: Emerging or adjunct evidence
Low-level laser therapy (LLLT)
LLLT devices, sold as combs, helmets, or caps, use red light at 630 to 660 nanometers, theorized to stimulate mitochondrial activity in follicle cells and modestly extend the anagen phase. A 2014 multicenter, randomized, sham-controlled, double-blind study found a statistically significant increase in hair counts versus a sham device in both men and women (American Journal of Clinical Dermatology, 2014), and a 2019 systematic review and meta-analysis concluded LLLT is potentially effective for adult androgenic alopecia, though effect sizes were smaller than minoxidil or finasteride (Journal of Cosmetic and Laser Therapy, 2019). Devices range from roughly 200 to over 1,000 dollars, are not covered by insurance, and require ongoing use several times weekly.
Topical caffeine
Caffeine is theorized to counteract testosterone's suppressive effect on hair follicle growth. The foundational data come from an in vitro study showing caffeine at 0.001 to 0.005 percent counteracted testosterone-induced growth suppression in isolated human hair follicles and stimulated growth on its own (International Journal of Dermatology, 2007). This is laboratory tissue-culture evidence, not a human clinical trial, and it is frequently overstated in marketing for caffeine shampoos. Human trials show some improvement in self-reported shedding versus control shampoos, but the trial base remains far smaller and lower quality than for minoxidil or finasteride. Caffeine topicals are low-risk and low-cost, but expectations should be modest.
Saw palmetto
Saw palmetto (Serenoa repens) extract is thought to mildly inhibit 5-alpha-reductase, similar to finasteride but far weaker. The key comparative trial randomized 100 men with mild to moderate androgenetic alopecia to saw palmetto 320 mg per day or finasteride 1 mg per day for two years. At the end, 38 percent of saw palmetto users showed improvement in the vertex scalp compared with 68 percent of finasteride users (Rossi et al., International Journal of Immunopathology and Pharmacology, 2012). It was well tolerated with no significant adverse effects, and is available over the counter as a modest option for people who want to avoid the sexual side effect profile of finasteride and dutasteride, accepting a meaningfully lower efficacy ceiling.
Tadalafil (off-label)
Tadalafil, a phosphodiesterase-5 inhibitor best known as an erectile dysfunction medication, increases blood flow through vasodilation, the same broad mechanism thought to partly explain minoxidil's effect. Small pilot studies and case series report improved hair density in patients taking tadalafil for other indications, but no large randomized trial establishes it as a hair loss treatment, and it is not FDA-approved for this use. Treat it as exploratory, used only under medical guidance for its approved indications.
What the evidence does not support well
Biotin supplements are heavily marketed for hair health, but clinical benefit has only been shown in people with an actual biotin deficiency, uncommon on a varied diet, and high-dose biotin can interfere with thyroid and cardiac lab tests. Most over-the-counter "hair growth" shampoos beyond 2% ketoconazole lack controlled trial evidence for androgenetic alopecia specifically. Scalp massage alone has only weak supporting data: a widely cited 2016 study involved just 9 healthy men without diagnosed hair loss, who massaged one side of the scalp daily for 24 weeks and saw increased hair shaft thickness but not hair count (Eplasty, 2016), a preliminary signal, not evidence that massage alone treats androgenetic alopecia.
Stacking treatments: combination therapy
The strongest evidence overall comes from combining mechanisms that target different parts of the problem: an anti-androgen like finasteride to reduce DHT signaling, minoxidil to extend the growth phase and improve blood flow, and microneedling to stimulate the wound-healing growth-factor response. Trials comparing combination regimens against any single component alone consistently find combination therapy outperforms monotherapy on hair count and density. This is where professional guidance matters most: stacking a prescription 5-alpha-reductase inhibitor with oral minoxidil multiplies the systemic effects that need monitoring, and a dermatologist can sequence additions and track response with standardized photography rather than guessing.
Timeline and realistic expectations
Hair follicles cycle slowly, so no treatment here should be judged before 3 to 6 months of consistent use, and full assessment of finasteride or dutasteride often takes closer to 12 months. Increased shedding in the first 2 to 8 weeks of starting minoxidil is a normal part of synchronizing the hair cycle, not worsening loss. The realistic goal for most people, especially those with moderate to advanced hair loss when they start, is stabilization plus modest to moderate regrowth in thinning areas, not a full return to a teenage hairline. Clinical evidence supports meaningful improvement in a substantial share of users, but no study here reports full reversal in advanced, long-standing baldness, and none should be described or marketed as a cure.
A practical, evidence-informed starting protocol

Prescription options, oral or topical finasteride, dutasteride, or low-dose oral minoxidil, carry real systemic effects and require bloodwork, blood pressure monitoring, or fertility counseling in some cases, and should only start after consulting a dermatologist. PRP and LLLT are reasonable add-ons for people with access and budget, but neither should be a first step ahead of the options above.
